Free eye test/dental check up (NHS)

A lot has happened in the last couple of weeks and I'm now on Universal Credit. I haven't been able to have an eye test for 2 1/2 years (I wear glasses), or a dental check up for nearly 3 years.
I've checked and it seems that I might be eligible to a free eye test (at Boots for instance) and a free dental check up (at a NHS dentist). I still need to register with a NHS dentist as the practice I was registered with has been bought by private dentists, and they actually closed down for lockdown number 2. I've contacted half a dozen practices today, they're all full!
For the free eye test or dental test, what evidence do they require? I don't have any paper correspondence from DWP or JobcentrePlus, it seems that everything is done online or over the phone.

I have been having some slight vision problems recently and had an eye test on Saturday. As a result I will be getting a pair of reading glasses.

I enquired about Universal Credit discounts & was told I would get £40 off my glasses & the opthalmologist generated an online voucher. I have paid my deposit & collect my specs in a couple of weeks. I have not had to provide any evidence to date.

They offer quick turn around and good prices with a reasonable range to choose from. Quality is acceptable, a bit plasitcy but do the job and are well made enough. Depends what sort of thing you're looking for really.

Thanks. I'm not bothered about the frame, although, if it's plastic (or nickel), it could, or would (nickel) give me contact dermatitis, which would make the glasses unwearable! I'm very fussy about the quality of the lenses though, I know you can get lenses which were not 'cut' correctly, and once I got someone else's prescription!

You can screen shot your claim to show your entitlement. We see it all the time at work. You can also get free prescriptions as well if you are entitled to free dental and eyecare.
