You can choose what to include. Some people include just earnings received some just do a daily,weekly progress of earnings on each site regardless of cashout.
Personally I include everything where I have requested payment though things sales I would include that when it actually reaches me.
Totally up to you what you do or don't count. Some people count vouchers others only cash.

I think different people do it in different ways. Personally, I wait until I cash out - you can't spend it until you have it

I prefer to post daily with the earnings made the day before rather than waiting out cashouts as I find it pushes me more to earn. If I've reached £9.50 I'll try and find something to give me that extra 50p to take it to £10.

I then use the Money Earning Diary section http://www.themoneyshed.co.uk/index.php?board=7.0 to keep track of what has actually been received.
