Minimum payment thresholds - your help needed

I'm trying to put together a spreadsheet of affiliate programs/advertising programs and their minimum payment threshold and their payment methods. This is only for things you can use on websites such as blogs, etc. Once it is complete, hopefully it can become a pinned post and be useful for everyone.

I've put it on Google Docs. Please feel free to fill in the gaps where you can, and add any programs not currently on there.
